2 Alarm Fire: 6202 Desmond Avenue btn. Chapman & Glenshade (Madisonville) Fire Box: 8376 Date: 11/4/06 Time: 04:58 a.m.
This is District Four-Fire Chief Robert McWilliams reporting on a two-alarm fire that happened on November 4, 2006, at 4:58 AM; the location was 6202 Desmond Avenue.
When fire units arrived, they found heavy fire coming from the first floor of a 1 to 2 story, two-family dwelling. As fire crews were preparing to enter the structure, what was believed to be a smoke explosion blew out the front two walls of the building's second floor.
Fire engulfed the structure and three firefighters narrowly escaped injury from falling debris; no firefighters injuries occurred.
Forty-two firefighters were on the scene for two hours extinguishing the fire. The Cincinnati Building Department was contacted and responded for demolition of the structure. Fire crews will remain on the scene throughout the morning.
All tenants escaped safely prior to the Fire Department's arrival. The fire loss is estimated at $150,000. The cause of the fire is currently under investigation by the Cincinnati Fire Investigation Unit.

1 Alarm Fire: 2007-2015 Dunlap Street "Emmert Grain Company" (Over The Rhine) Date: 11/07/06 Time: 03:08 p.m.
This is District Chief Howard Reed from District One of the Cincinnati Fire Department reporting on a one-alarm fire that occurred at the Emmert Grain Company. The company is located at 2007-2015 Dunlap Street in Over-The-Rhine. At 3:08 p.m. on 11/07/06 fire companies were dispatched for smoke in the area of McMicken and Dunlap.
Firefighter found heavy smoke coming from the roof area of the Emmert Grain Company. The fire was found in a bin that contained grain where it was confined and extinguished.
It took Firefighters 30 minutes to bring this incident under control with no injuries to civilian or firefighters.

2 Alarm Fire: 4828 Glenway Avenue (Price Hill) Date: 11/07/06 Time: 11:24 p.m.
This is District Chief Howard Reed from District One of the Cincinnati Fire Department, reporting on a two-alarm fire that occurred at 4828 Glenway Avenue, which is located in Price Hill.
Firefighters were dispatched on 11/7/06 at 11:24 p.m. to a report of a fire in the basement of a four-story brick multi-unit apartment building; upon our arrival, we found heavy smoke coming from it. Firefighters made an aggressive interior attack, bringing the fire under control in less then 15 minutes. The fire was contained to the storage area in the basement with no vertical fire extension.
There were working smoke alarms sounding on our arrival, which alerted the occupants, enabling them to escape prior to our arrival. No civilian or firefighter injuries were reported.
The cause of the fire is undetermined and is being investigated by Fire Investigator Paul Alloway.
The fire caused approximately $30,000 damage to the structure and the contents in the basement.

3 Alarm Fire: 150 West 5th Street "Millennium Hotel" (CBD) Date: 11/12/06 Time: 20:06 hours
This is District Chief Fredrick Prather of the Cincinnati Fire Department reporting on a three-alarm fire that occurred at 150 West 5th Street, the location of the Millennium Hotel.
At 08:06 PM, firefighters responded to a fire alarm, and upon arriving, they found hotel guests evacuating the structure, which had heavy smoke coming from its first floor. A grease fire occurred on one of the cooking appliances in the hotel's kitchen, causing the smoke.
Prior to the Fire Department's arrival the bulk of the 408 guests had exited the structure, as well as the hotel's 60 employees. The hotel guests and employees were provided temporary shelter at the Duke Energy Center
Firefighters initially performed primary and secondary searches of the structure and firefighting efforts to extinguish the fire that had extended into the structure's ductwork, which was above a cooking appliance. To extinguish, the fire firefighters were required to open up the ductwork. Firefighters were able to complete the extinguishment of fire in one hour and 30 minutes.
The fire caused approximately $300,000 damage to the structure and its contents.

3 Alarm Fire: 45 W McMicken Avenue btn Race & Findlay (Over The Rhine) Date: 11/18/06 Time: 01:50 a.m.
This is Cincinnati Fire District One Chief Steven Phillips reporting facts pertaining to a three-alarm fire that occurred last night in Over the Rhine.
On Saturday, 11/18/06, at 1:50 A.M, patrolling police officers driving by discovered flames erupting from an occupied four-story apartment building at 45 W. McMicken Avenue. Firefighters arrived moments later to find heavy fire on three floors with numerous occupants trapped by the flames.
Through the heroic efforts of the firefighters, at least 10 people were rescued either by ladder or carried down the interior stairs. Despite the rapidly spreading flames and the collapse of a stairwell, all occupants were saved.
One firefighter was injured and is being evaluated at the hospital. Thirty people have been displaced and are being assisted by the American Red Cross.
The cause of the fire is under investigation. The loss is estimated at $100,000. Smoke detector performance is also under investigation.

1 Alarm Fire: 2857 Cypress Way (Pleasant Ridge) Date: 11/20/06 Time: 03:40 p.m.
This is District Chief Alan L. Sedam, reporting on a one-alarm fire this afternoon at 2857 Cypress Way in Pleasant Ridge.
A 911 call at 3:40 PM, reported an apartment on fire. Upon arrival, Engine 8 and Ladder 31 reported heavy smoke and fire from a one-story Apartment type unit. Firefighters found extensive fire in Apartment 2. An aggressive interior attack was mounted and the fire quickly brought under control.
All civilians exited the structure before our arrival. There were no civilian or firefighter injuries. There were working smoke alarms on every floor. Smoke alarms do save lives!
The cause is under investigation and the loss is estimated at $70,000

One Alarm Fire: 4505 Eastern Ave Date: 11/23/06 Time: 11:10 a.m.
This is District Chief Alan L. Sedam, reporting on a one-alarm fire that occurred this afternoon at 4505 Eastern Avenue. A 911 call at 11:10 AM reported a second floor fire at that location.
Upon arrival Engine 18 and Ladder 18 reported heavy smoke and fire coming from the second floor of a two-story multi-dwelling. Firefighters found extensive fire on the second floor. An aggressive interior attack was mounted bringing the fire quickly under control; the fire cause is undetermined.
All civilians exited the structure before our arrival; there were no civilian or firefighter injuries.
There were working smoke alarms on every floor; smoke alarms do save lives!
The cause is under investigation and the fire loss is estimated at $40,000.

1 Alarm Fire: 3543 Van Antwerp Avenue (Avondale) Date: 11/24/06 Time: 06:58 p.m.
This is Cincinnati Fire District 3 Chief Randal T. Freel Sr. reporting on a one-alarm fire, which occurred at 3543 Van Antwerp Avenue in Avondale, the property is an Elderly facility.
Fire personnel were dispatched for a building fire. Fire companies confirmed a working fire in a three-story brick structure. The first floor had smoke showing from the rear of the building. An interior fire attack was initiated along with a primary search. The fire was brought under control within a short period of time.
The building's sprinkler system contained the fire; also the primary search confirmed the building to be all-clear. The occupants were sheltered in place. The doors to the other units were closed after it was determined that the fire was under control and smoke would not be a danger.
The fire cause is under investigation at this time. The estimated loss was set at $10,000 damage to structure and its content. There were 25 fire personnel utilized at the incident. Firefighters were on the scene for approximately two hours and 30 minutes. There were no injuries to civilians or fire personnel.

1 Alarm Fire: 2531 Burnet Avenue (Corryville) Date: 11/25/06 Time: 01:38 a.m..
This is Acting District Two Chief George Hornback of the Cincinnati Fire Department reporting on a one-alarm fire that occurred at 2531 Burnet Ave, located in Clifton.
Fire companies responded to a report of a fire in an apartment building at 01:38 a.m. on 11/25/06.
Upon our arrival, we found fire coming from the top floor of a three-story brick multi-dwelling. In addition, we found one adult and one child trapped in the apartment on fire. They were immediately escorted out of the building and transported to the hospital for observation.
Firefighters made an aggressive interior attack, bringing the fire under control in less then 10 minutes. Two people were rescued from this structure.
A working smoke alarm was found in the apartment. The cause of this incident was food left unattended on the stove. The fire caused approximately $5,000.00 loss the contents and structure.

1 Alarm Fire: 1275 Elberon Ave (East Price Hill) Date: 11/30/06 Time: 08:55 p.m.
This is District Two Fire Chief Gary Scott reporting on a one-alarm fire, which occurred at 1275 Elberon Avenue on November 30, 2006.
At 8:55 pm fire companies responded to a report of a building on fire. While en-route, fire personnel reported smoke-in-the-area. Upon arrival, they confirmed a working fire in a one-story single-family dwelling. An interior attack on the fire was initiated, along with a primary search of the entire structure.
A small fire was located in the basement area and quickly extinguished. The primary search confirmed the building was all clear. Neighbors in the immediate area reported the building has been vacant for several months. A fire investigator was called to the scene; at this time the fire cause is "undetermined" pending further investigation. Fire loss was set at $1000. There were 28 fire personnel utilized at this incident. There were no injuries to civilians or fire fighters as a result of this incident.

2 Alarm Fire: 538 Rockdale Avenue btn Harvey & Washington (Avondale) *1 CIVILIAN FATALITY* Date: 12/02/06 Time: 15:00 hrs
This is Cincinnati Fire District One Chief Steven Phillips, reporting facts concerning a two-alarm fire in Avondale on Saturday 12/02/06.
Firefighters responded to a reported fire at 3:00 PM at 538 Rockdale Avenue. Residents of a three-story apartment building were fleeing the burning structure and reported there was a man in the window in the rear of the building.
A second fire alarm response was requested because of the lives at risk. Cincinnati Firefighters entered the burning apartment and quickly extinguished the fire. They found one male victim deceased in the burned out apartment.
Firefighters assisted approximately 20 other people from the smoke filled hallways. Forty-five firefighters were on the scene for several hours. Fire damage is estimated at $30,000. The cause of the fire is under investigation. Smoke detector performance is also being evaluated.

2 Alarm Fire: 951 Dana Avenue btn Clinton Springs & Dakota (North Avondale) Date: 12/03/06 Time: 14:26 HRS
This is District Four Fire Chief, Rob McWilliams reporting on a two-alarm fire that occurred at 951 Dana Ave in Avondale. The fire happened on December 3, 2006 at 2:26 PM.
When fire units arrived, they found fire and heavy smoke coming from the first floor of a two-A brick, two-family dwelling. Fire quickly progressed to the second floor and attic areas, resulting in a second fire alarm to be requested. Fire companies quickly advanced to the fire areas bringing the main body of fire under control within twenty minutes.
Fifty-five firefighters were called to the scene; and final extinguishment and overhaul kept units on the scene for over two hours.
All tenants safely escaped the fire. The fire loss is estimated at $100,000. The cause of the fire is currently under investigation by the Cincinnati Fire Investigation Unit.
The American Red Cross is caring for two displaced families.

1 Alarm Fires: 519 Livingston Street btn John & Bauman (West End) Date: 12/07/06, 12/08/2006 Time: 11:27 p.m.
This is District Chief, Fredrick Prather, reporting on a fire at 519 Livingston Street, the location of a vacant building that was being demolished.
The Cincinnati Fire Department is on the scene defensively fighting the fire from outside the structure. Fire companies cannot safely enter the structure because of building conditions. Fire crews will be on the scene for an undetermined amount of time.
The Cincinnati Fire Department has also performed air monitoring and has found the air to only contain the particulates that would result from a typical structural fire. The Fire Department will continue to assess the situation.

This is District Four-Fire Chief Robert McWilliams reporting on a one-alarm fire that occurred on December 18, 2006 at 4:39 PM at 2625 Moorman Avenue in Walnut Hills.
When fire units arrived, they found heavy smoke from the third floor of a three-story dwelling. An aggressive fire attack contained the fire to the third floor and attic area.
Twenty-eight firefighters remained on the scene for over two hours extinguishing the fire and overhauling. Five residents, three adults and two children were displaced and are being assisted by the Red Cross. Damage is estimated at $15,000.
The cause of the fire is currently undetermined and being investigated by the Cincinnati Fire Investigation Unit.

2 Alarm Fire: 87 Juergens Avenue btn Green Hilll & Lauk (Avondale) Fire Box No. 2381 Date: 12/24/06 Time: 10:47 p.m.
This is Acting District One Fire Chief George Hornback reporting on a two-alarm fire, which occurred at 87 Juergens Avenue in Clifton.
A 911 call for a house on fire across the street from 68 Juergens was reported at 10:47 PM.
While fire companies were responding, the 911-call center received reports of people trapped in the building. Upon arrival, fire units confirmed a working fire in a two-story frame building with heavy fire showing on the first and second floors of the structure.
An aggressive interior attack of the fire along with a search of the building, one occupant was still inside the structure. The victim was removed from the building and was provided appropriate care by fire personnel. The fire was brought under control within 30 minutes of initial dispatching.
A fire investigator was called to the scene to determine the cause of the fire. At this time the cause is undetermined. The loss was set at $60,000. There are no reports of firefighters injured. There were 45 fire personnel at this incident.
