Care-giver and 1 child exited the building and contacted 911 Emergency to report fire in the structure. Small fire contained to the kitchen area by an aggressive interior fire attack initiated by CFD personnel. Fire cause determined to be a faulty dishwasher and damage is estimated at $6,000. Working smoke detectors were present within the residence.

NUMBER OF PEOPLE DISPLACED: N/A SMOKE DETECTORS PRESENT: N/A INCIDENT DESCRIPTION: During the preparation for demolition of the old Hopple Street overpass, a catastrophic pancake collapse occurred of a section of that bridge onto the southbound lanes of I-75. A tractor trailer struck the collapsed section as the bridge hit the ground. The truck driver sustained non-life threatening injuries and was transported to an area hospital for evaluation. Unfortunately, a construction worker was killed in the collapse. The name of that worker is not being released at this time, pending notification of family The southbound lanes of I-75 will be shut down indefinitely during the investigation and removal of the collapsed section of roadway. CAUSE (IF KNOWN): Outdoor Fire # OF PEOPLE DISPLACED: 6 adults, 9 children SMOKE DETECTORS PRESENT: Unknown BRIEFLY DESCRIBE INCIDENT: E-24 was originally dispatched for an outdoor fire, upon arrival they discovered a fire in the driveway that was extending into the structure. E-24 requested the full 1 alarm compliment upon discovery. Fire extended into the garage causing severe damage to the electrical meters and wiring located just inside the doors. Fire melted the windows in the apartment above, which was unoccupied at the time. There was a small amount of fire extension into the sofit above the fire area. Companies brought the fire under control within 10 minutes, but due to the damage to the electric, Duke Energy had to cut power to the building. A total of 6 adults and 9 children were displaced and are being assisted by the Red Cross. 1 civilian was transported by M-24 for possible smoke inhalation injuries. No Fire Personnel were injured.

One adult male was displaced and is staying with a friend in the same building. All other occupants in the building were able to return to their units.

Firefighters conducted an aggressive attack and extinguished the fire within minutes from their arrival on scene and prevented the fire from spreading. Damage was contained to the kitchen area only and is estimated at $8,000. Less than a full one-alarm compliment was used at this incident and there were no reported injuries.

Firefighters had difficulty gaining access to the building due to the metal security mesh covering all the first floor windows. Police also stated that there were numerous squatters that lived in the building. Due to the size of the building and the difficulty in gaining access, a second alarm was requested. An aggressive interior attack was initiated and the fire was extinguished in approximately 30 minutes. There were no civilians found in the building during the primary or secondary searches, One firefighter received a minor burn. He was treated at the scene and remains on duty. The fire is under investigation by the Cincinnati Fire Department Investigation Unit. Firefighters were on the scene for just over two hours completing overhaul.

E-38 after leaving quarters viewed a large column of smoke in the area. Upon arrival, found heavy fire conditions from beneath road surface, located up inside the I-beam bridge spans that cross the Millcreek waterway. The roadway was immediately closed to traffic. Millcreek canal has extremely steep, concrete flood walls and a narrow ledge to crawl on for access underneath the bridge. With icy conditions forming on these steep surfaces, firefighters had trouble getting water to the area where the main fire was burning. Several explosions were occurring, 1 being a propane cylinder that bleved and rolled approximately 40’ down the flood wall into the bridge support where it was extinguished. With the use of rope lines, firefighters were harnessed and lowered down the walls into a position where water could effectively be applied from a safe location, and were able to extinguish after approximately 30 minutes. The Ohio Department of Transportation and Cincinnati Highway maintenance were notified. The intense heat caused spalling to the concrete below the main deck and twisted a steel cross member. The bridge was deemed safe to re-open by ODOT at approximately 2045 hours, but inspectors were going back on Friday morning for further investigation. The cause of the fire at this time is still being investigated. No injuries occurred.

FIU Car 514 is investigating the cause of the fire, which is undetermined at this time. The Red Cross was notified to assist 3 adults and 3 children from the multiple apartments damaged by fire.

The workers and management staff of US Bank maintained their calm demeanor, cordoned and sealed off the area, isolated all personnel and contacted Emergency 911 where the Cincinnati Fire Department Hazardous Materials compliment was immediately dispatched. Upon arrival, the CFD Hazardous Materials Units under the supervision of Special Operations Chief Thomas C. Lakamp donned protective gear and entered the isolated area for substance evaluation. After a complete and thorough chemical analysis it was determined to be a non-hazardous material. The FBI Terrorism Unit and the US Postal Inspector were notified and representatives from both agencies responded to the scene for investigation and potential prosecution.

The fire was rapidly extinguished by CFD personnel preventing extension beyond the room of origin. The structure is habitable in spite of the moderate smoke and cosmetic damage.
